Health Hero School Vaccination Clinic for Students
The Health Hero school vaccination program will offer the required Tdap vaccine and/or recommended vaccines to students in grades 5-6 and students in other grades that need the required Tdap vaccine for school attendance. This is a voluntary school vaccine program and permission forms will be sent home with all 5-6 grade students (and other students that need the required Tdap vaccine) on Monday- February 11, 2019, and must be returned by Friday-February 15, 2019.
This vaccine program is for students with Medicaid, AllKids, private insurance or no health insurance. If the student has PEEHIP Blue Cross and Blue shield that begins with EDU they are not covered by this program.
The Health Hero Clinics will be February 26, 2019, for East End Schools and February 27, 2019, for West End Schools and Rogers. Please call your school nurse if you need additional information.
Health Hero Permission forms must be completed in full using black ink.
