Alabama Department of Public Health will offer the PEEHIP Wellness Screening and Flu vaccine to employees on their school campus during the months of October and November.

The wellness screening is a requirement for employees and spouses covered under PEEHIP Blue Cross and Blue Shield Insurance in order to avoid a monthly increase in insurance premiums.  PEEHIP Blue Cross and Blue Shield Insurance has a contract number that begins with EDU. 

All appointments for the PEEHIP Wellness Screening must be made on-line on the ADPH website. (Instructions are listed below.) Employees may schedule the wellness screening appointment at their school campus or at the Health Department.  (School Dates are listed below.) Employees may also schedule the wellness screening with their physician using the required form.  There will also be additional school wellness screening opportunities in the spring.

The Flu vaccine is available but not a requirement of the wellness screening.  The Flu vaccine employee consent form is a carbon copy and must be completed at the time of the wellness screening.  Staff that only want to participate in receiving a Flu Shot do not need to make an appointment.

How to make an appointment on-line for the PEEHIP Wellness Screening

Employees can now access and sign up online for their Wellness screening. The direct link is below:

Once there, you can click the PEEHIP option on the left.

You can search by dates on the left but there is also a search option above the listed sites.

Type Lauderdale in the search box it will pull up all PEEHIP Lauderdale County sites and also screening dates for the Health Department.
