Health Hero Spring Vaccination Clinic for Students
Health Hero will offer the required Tdap vaccine and other recommended vaccines (MCV4, HPV, and Meng B) to students in grades 5, 6 and 12 with parent consent forms. This vaccine clinic is for students with private insurance, Medicaid, Allkids, or for students without health insurance. Students with PEEHIP Blue Cross, Blue Shield that have contract numbers that begin with EDU are not eligible for this program.
This is a voluntary school vaccination program and consent forms will be sent home with students in grades 5, 6, and 12 beginning Monday, February 20, 2023 and must be returned to the school nurse by Friday, February 24, 2023. All insurance information must be completed in full on the consent form. The date for this clinic is March 2-3, 2023 at each school campus. Students in other grades that are missing the required Tdap vaccine may receive it on this day with a parent consent form.
