All Freedom of Choice forms for the 2021-2022 School Year are due now. If your child is a returning student or new to the district, forms are due to your current school. Even if you think your child may attend a different LCSS school next year, turn the forms into your current school.
This is for this year's K-11 grade students. PreK/TOPS/K students will receive forms at registration and if selected for the programs.
Click here for a fillable form (to be typed in or downloaded) -- print and return to your current school.
We are excited as we are already looking forward to next school year!
