2025-2026 Returning Student Enrollment
This form is for students who were in grades K-11 at a Lauderdale County School during the 24-25 school year and plan to return to the same school they attended. These forms will require a Snapcode. You will receive a letter sent home with your student and/or an email with the Snapcode.
NOTE:
The Alabama State Department of Education Health Assessment Record is required to be completed on each student at the beginning of the school year and is included in online Enrollment. In the event online Enrollment is not completed, a paper copy will be sent home for the parent/guardian to complete and return. (2-page form located at the end of the 2025-2026 Parent/Student Handbook.)

2025-2026 NEW Student Enrollment
This form is for students who wish to attend a Lauderdale County school in the 25-26 school year and did not attend in the 24-25 school year OR your child was in Pre-K or Preschool in the 24-25 school year.
**A unique verification code will be provided to you via email once you have accepted your placement in a TOPS PreSchool or Pre-K class.

REQUIRED DOCUMENTS FOR NEW STUDENT ENROLLMENT:
- Age verification document (Birth Certificate or Passport)
- Unexpired ALABAMA immunization card or exemption card
- Student's Social Security Card (optional)
- Proof of Residency: must be in parent/spouse/legal guardian's name and ONLY in the form of: a current utility bill (Florence City or city of residence), or ALL pages to a current valid verifiable rental/lease agreement, or verifiable current mortgage statement or contract on a home that includes the signature page.
- Parent/Legal Guardian photo identification (driver's license)
