Freedom of Choice Form 2021-2022

The Lauderdale County School System is using the Freedom of Choice form for registering students for the next school year (2021-2022). This form is taking the place of in-person or virtual (online) registration. 

Parents/Guardians, please fill out one form for each child. All forms should be returned to your current school. Even if you think your child(ren) may attend a different school next year, turn the forms into your current school.

If you are new to the Lauderdale County School System, you may also register using this same form.
